ピンクスリップの再発行について

車を個人売買で売るときに、ピンクスリップが紛失していることに気づき、再発行しようとしたのですが、その申請用の紙でTRANSFERの手続きができることに気づきました。REG２２７と言う紙です。
しかしその中の1つの項目がわかりません。
だれか助けてください。
（５）のLEGAL owner of record release of ownership and/or interest

このFORMを書かなければ、車を売れないので困っています。

おっと！ここは、あなたが書くのではなく、公証人（NOTARY PUBLIC)に書いてもらうのです。
（"SIGNATURE MUST BE NOTARIZED"って書かれていますよね？　サイン証明が必要なのです。）
手順としては、用紙をNOTARY PUBLIC（ご近所にもきっとあると思います。）に持ち込み、公証人の前で、
あなたが"COUNTERSIGN"をし、そこから下は、公証人が書き、ハンコ(OFFICIAL SEAL)を押して、サインします。
