Submission of Inspection Certificate

On the PDF form mentioned above,

it says "Signature by doctor", but it is not clear from the signature whether the person is a doctor or not, and anyone's signature would be acceptable.

Also, "An imprint of a seal" on the right side of the signature is not acceptable, as no one in the U.S. has a stamp, even in a doctor's office, and anything that can replace it is acceptable. To take it to the extreme, you could cut out the name of the doctor's office from the doctor's office business card and stick it on the card with glue.

Booked online here and you can find it by putting app. passport no other info is needed.
The result was chosen 24 hours later for my wife.
I took it 72 hours ago.

Why do you need the doctor's signature ？ There was no doctor's signature on the negative certificate. You can get a document from the Ministry of Health and Welfare from the airline company at the airport. You can submit it in Japan and have an antibody test, and if you can prove negative by saliva, you can enter the country. Of course, when you leave LAX, you also need the negative certificate from LAX. The same applies to the time of entry into Japan.
