I-485 Package

I-130のApproval Noticeが届いたので、I-485の申し込みをするところです。
弁護士なしで、主人と2人で全部しています。主人はアメリカ市民です。
I-864、I-765、 I-131と G-325の他に、I-693 Medical Examinationも提出しないといけないんでしょうか?
主人と私が何回かUSCISに電話して聞いたんですが、必要ないと言われたり必要だと言われたり、人によって言う事が違います。インターネットでの色んなForum も読んでみたんですけど、雇用ベースの場合だけ必要だとか、全部ばらばらなのでわかりません。
後、日本からBirth Certificateも取り寄せないといけないんでしょうか?
ご存知の方、経験者の方教えて下さい！

ダウンロード出来るので、じっくり読んでみてください。

今朝またUSCISなどに問い合わせたら、やっぱりMedical Examは受けないといけないみたいですね。Birth Certificateも日本から取り寄せないといけないみたいです。
トピック消せないみたいなのでこのままほっておきますが、間違ってたら教えてください。

永住権申請中です。
私はI-693、提出しました。
Birth Certificateは本籍がある市役所から戸籍謄本を取り寄せて自分で英訳しました。

戸籍を自分で訳すのに問題はありませんが
他に日本語のわかる方のサインも添える様に言われました。
